To serve you best, a  comprehensive wellness assessment is required before your initial consultation. A properly filled out assessment form will help Pauline understand your health history, lifestyle, mood and energy levels better. It also helps in ascertaining your chief medical concerns in detail, gives a thorough understanding of your diet, your fundamental beliefs, and overall level of fitness.

Pauline will use the information collected in establishing a cause for your symptoms and condition, creating a way to track your improvements and (when required), selecting the appropriate herbs.  In short, the assessment form helps Pauline attend to your unique needs and what herbs will be safe and effective for you. The information will help offer guidance for your nutrition and other areas of wellbeing too.

The followings are needed before the initial consultation – any blood work, previous testing, or image reports. Provide a list of medications and or supplements. The analysis of your assessment form will be used to create a Client Comprehensive Protocol, which will be discussed with you in person or over the phone. Your initial consultation may take up to 60 minutes. During the discussion, all your questions and concerns will be addressed, and your health goal will be clarified.

If your wellness goal spans many months, there will be need for follow-up visits. During these visits, needed revisions to your wellness plan will be discussed. New concerns will also be addressed.
